LEAD: Avery International Inc. said it planned a restructuring in 1988 that would affect about 1,000 workers, cut the revenue base by 9 percent and result in a $25 million write-off and an unspecified loss in the fourth quarter. Avery, a manufacturer of self-adhesive labels and office products based in Pasadena, Calif., said the overhaul would mean the first earnings decline in 12 years.
